有不少的准妈妈,对于在怀孕期间治疗牙齿有诸多的恐惧,例如:怀孕期可以看牙吗?照X光会不会对胎儿造成伤害?可以服用牙科药物吗?麻醉药会不会对胎儿造成伤害?其实根本之道,就是怀孕前就养成良好的口腔卫生习惯,并于怀孕前解决所有牙齿问题。

  生一个孩子,坏一颗牙?

老一辈的人会有“生一个孩子,坏一颗牙”的观念。其实,如果在怀孕前建立正确的口腔卫生观念及习惯,同时在怀孕期间保持良好的口腔卫生,这种现象就不会发生。

有人误以为孕妇牙齿的钙质,会被腹中的胎儿吸收而造成本身牙齿钙质的流失。事实上,钙在牙齿中是以结晶状态存在,发育完成后的牙齿不会参与体内钙的代谢。不过骨头中的钙会被腹中的胎儿吸收,所以仍要增加钙的摄取,以免妈妈在日后发生骨质疏松症。

  牙周病会影响胎儿健康

牙周病主要是由厌氧菌感染所引起,细菌本身及其他代谢产物,在牙龈发炎时会释放出来一些炎性因子,可能导致孕妇血糖代谢不良,甚至早产;此外,孕妇因牙痛而进食不易,会导致营养不均衡,也会间接影响胎儿的健康。

1996年美国牙周病学会研究报告也发表:怀孕妇女患有严重牙周病者发生流产、早产或新生儿体重过轻的几率,为一般口腔健康良好孕妇的七倍。所以怀孕妈妈们可千万不要轻忽。

         孕期牙科治疗的六大疑问

孕妇的情绪不易控制,如果能建立一个良好的医病关系,可以减少孕妇的焦虑。必要时,可以先由妇产科及内科医师会诊孕妇及胎儿的状况后,再去找牙医师诊察比较好。而以下一些准妈妈常会问的牙科问题,我们在此为您解除疑惑:

1 有牙齿问题一定在怀孕前就处理好吗?

准备怀孕前就要建立一个健康的口腔环境——询问牙医师正确的刷牙法(用贝氏刷牙法,避免横向的“锯”牙)、用牙线去除牙菌斑的附着,及定期洗牙。要认真的刷牙并将口腔中的蛀牙治疗好,以免到了怀孕期,更不愿到牙科就医,因而延误了治疗蛀牙的时机,使得产后牙齿损坏的机会大增。

2 孕妇在治疗牙齿时照X光片,会不会对胎儿造成伤害?

一般而言,只要总曝量不超过5~10雷得,就不会出现先天性畸型。而牙科放射线的剂量,全口头颅片放射量是0.004雷得,全口14张牙根尖片也只有0.00001雷得,再加上每次照射X光时都会用铅衣防护,几乎可以完全保护孕妇及胎儿免于受到辐射。而且牙医师只有在治疗上必要时才进行照射,安全上无庸顾虑。

3 怀孕后才发现有蛀牙怎么办?怀孕期可以看牙吗?

一般简单的洗牙及补牙在妊娠期间都可以接受治疗,但如果孕妇属于情绪紧张型,妊娠前期比较有流产之虞;妊娠后期则比较怕会因为仰卧的姿势压迫静脉血管,预防之道为坐起来的时候先休息一下,避免突然站立导致晕眩(因姿势性低血压的关系)。等到妊娠中期,孕妇的状况最稳定,肚子也不会太大,大部份的牙科治疗(如洗牙、牙科补缀、根管治疗等)都可以在这个阶段完成。但是牙科手术(如:牙周翻瓣深部治疗、拔除智齿、牙根尖手术)因为手术时间较长,在情绪上对孕妇造成的压力也比较大,所以建议延后到产后再做治疗。

4 孕妇可以服用治疗牙齿的药物吗?打麻醉药会不会对胎儿造成伤害?

抗生素:除了四环霉素、氯霉素及链霉素外,一般而言,牙科抗生素对孕妇来说是安全的。因为感染可能造成菌血症或败血症,对胎儿的危害要比抗生素通过胎盘对胎儿的危害更大,所以必要时仍会建议孕妇服用。

止痛药:一般常用的止痛药,如阿斯匹灵及普拿疼,到目前为止妇产科医师也都认定是安全的;麻醉性止痛药(如:吗啡)不会对胎儿造成永久性的伤害,但是会抑制中枢神经系统,而且长期使用,会造成成瘾性,不过牙科医师不会开这种处方。

麻醉剂:牙科局部麻醉剂只要不过量,都可安全使用。

5 婆婆说坐月子不要刷牙,只要漱口就好,这样正确吗?

这是错误的观念及习俗,因为漱口只能将口内大块的食物残渣漱掉,但是附着在牙齿表面的牙菌斑(包括食物残渣、细菌及其分泌物)一定要靠刷牙才能清除干净。如果在坐月子这段期间都没有刷牙,很快的在二周内就会造成牙龈炎,牙结石也会形成,牙菌斑一旦钙化形成牙结石,就没有办法靠刷牙的方式移除,一定要用超声波震荡的力量才能清除牙结石。

6 孕妇何时看牙较好?

妇女在怀孕之前看牙最好,基于孕妇的舒适与牙科治疗的安全,应尽量避免在怀孕的初期与末期作治疗,牙齿若有不适可请牙医做暂时性且不太辛苦的处理。非作不可时,可于怀孕中期治疗。如果必须接受牙科治疗,也请放松心情,安心接受治疗,因为牙医师会衡量治疗上的必需性,尽量减少X光曝露量及不必要的药物和感染机会。

*怀孕第一期(13个月)有时怀孕初期孕妇并不知道,而这时也正是胎儿重要器官(手脚、脑脊髓神经系统、牙齿)形成的三个月,若在此时服用不当药物,或是接受过大剂量的放射线,可能会造成自然流产的威胁,也有可能形成畸型儿,所以大多数牙医师在此时只作紧急处理,不会作太过激烈的处置。

*怀孕第二期(46个月)可做侵犯性不大的牙科处理,在适当的保护下(穿铅衣)也可以照射X光片。

*怀孕第三期(79个月)此期只适合作紧急处理,而且要避免维持头下脚上的姿势太久,以免突然坐起来的时候,造成姿势性低血压。

*哺乳期:最近许多专家提倡哺喂母乳,母乳对于孩子的健康最好,但是因为许多药物会经由乳汁分泌,被婴儿摄取进入体内,所以如果有哺乳时,也要告知医师。如果非用药物不可,最好服药跟哺乳时间间隔4小时以上,减少药物在乳汁中的剂量,甚至要牺牲喂奶以免影响婴儿。

         准妈妈拥有一口好牙的方法

母亲在怀孕期要摄取充分的营养,蔬菜、水果、米饭、鱼、肉、蛋、奶类皆要均衡摄取,才能供应胎儿牙齿所需的钙质、磷质、维生素等等。特别是钙质(如:牛奶、小鱼之类),不但有助于胎儿发育及建立坚固牙齿,也可维护妈妈自己的健康。同时怀孕期间也要比平常注重口腔卫生。

牙医师建议最好在怀孕前就养成定期检查的习惯,如果怀孕前没有检查牙齿,最好在怀孕中期找个时间作牙齿检查及洗牙,以免口内有严重的蛀牙及牙周病,等到发生比较严重的症状(如引起牙痛或身体不适)时再寻求治疗,对胎儿反而更不好。

Claiming around 610,000 lives each year, heart disease is the No. 1 killer of both men and women in the U.S. Did you know that research has found a link between this deadly disease and the health of your gums?

Treating gum disease can go a long way in lessening the impact of coronary artery disease (CAD), according to a 2014 study in the American Journal of Preventive Medicine.1 Patients who were treated for gum disease have lower medical costs and fewer hospitalizations for CAD.

Does gum disease cause heart disease?

Despite the link between gum disease and heart disease, it’s not clear whether one actually causes the other, according to the American Heart Association.

The two conditions have some of the same risk factors, including smoking, poor nutrition and diabetes.

But even in non-smoking populations, researchers have found a strong correlation between the oral condition and heart disease. One hypothesis is that bacteria from the oral cavity spread throughout the body, worsening other inflammatory conditions, like heart disease, rheumatoid arthritis and type 2 diabetes.

Prevention is the best medicine

Regular healthy habits can lower your risk of both gum disease and heart disease. And if you already have one or both of these conditions, these strategies can help reduce their impact.

  • Brush and floss regularly. To remove plaque-forming bacteria, brush for at least two minutes, twice a day, and don’t skip the floss.
  • Choose a healthy diet, rich in essential nutrients (especially vitamins A and C). Reduce or eliminate sugar and starches.
  • Avoid cigarettes and smokeless tobacco. Even smokeless tobacco can destroy your gums and increase your chance of heart disease.
  • Visit the dentist for regular cleanings and exams. Your dentist can identify signs of systemic illness and catch gum disease early on. Always tell your dentist your medical history, current conditions and any medications you’re taking.

口腔健康和身体健康息息相关,妥善照顾牙齿与牙龈的健康,不仅能让你微笑时展示雪白皓齿,更能提升整体健康,减低罹患严重疾病的风险,甚至能在老年保有好记忆。所谓“健康的身体来自健康的口腔”,越来越多的科学证据证明此言确实不虚。

根据调查,17%的成年人从不用牙线洁牙,25%的35-59岁成年人有蛀牙却不治疗。教导孩子正确的牙齿与牙龈保健知识永远不嫌早,孩童时期养成良好的健康习惯,对于成人后的健康有很大的助益。倘若你噎开始轻忽刷牙、用牙线清洁牙齿和定期看牙医等良好的口腔卫生习惯,千万别忘了你的行为是孩子学习模仿的对象。
据WebMD医疗网站报导,健康的牙齿和牙龈对于促进整体健康有6大益处:

(1)帮助糖尿病患稳定血糖值
大部份专家认为,糖尿病与口腔健康的相关性是双向互为因果,一方面糖尿病控制不良会提高牙龈疾病的风险,另一方面有牙龈疾病将使得血糖值更难控制。其它与糖尿病有关的口腔问题如蛀牙、口干燥病、感染真菌如口腔念珠菌病(鹅口疮)和牙科手术后复原速度缓慢。糖尿病患者若能维护口腔健康,降低罹患牙龈炎的风险,便能协助控制血糖值。
糖尿病患抵抗感染的能力较差,牙龈感染可能导致严重的牙龈疾病。自我检查口腔时若发现发炎或出血症状,应立即到牙科就诊。专家建议,每6个月或甚至每3-4个月就应到牙科进行专业洗牙。同时必须让牙科医师知道你是糖尿病患,以及你目前服用的处方和非处方药。如果牙龈问题持续恶化,就必须找牙周病专科医师。


(2)可能减少心脏病风险
美国牙周病学会指出,相较于没有牙周病的人,牙周病患者罹患冠状动脉疾病的机率高一倍。此外口腔细菌附着在冠状动脉的脂肪斑,形成血液凝块引发心脏病。专家们虽然还不能明确指出牙龈疾病与这些严重疾病之间的因果关系,但是诸多研究显示,牙龈疾病引发的慢性炎症一直和心脏病、血管堵塞和中风等心血管疾病有关连性。即便是一般的口腔问题也可能提高心脏病的风险。这些研究报告也指明,维持口腔健康对于维护人体的全面性健康有助益。

(3)提升自尊与自信
蛀牙与口腔疾病不只影响牙齿的美观,还会造成口臭,进而影响自信、自我形象与自尊心。美国牙周病学会调查,至少23%的30-54岁妇女和44%的55岁以上妇女有严重的牙龈疾病或牙周病,这是造成牙齿脱落的主要原因。除了不美观外,一旦牙周细菌进入血液中,可能引起慢性发炎。青春期、怀孕期和更年期妇女更应注意牙龈健康,由于荷尔蒙改变可能导致牙龈疾病快速发展,因此牙周病医师史耐德(David Schneider)建议妇女每天要记得以牙线洁牙。


(4)降低早产风险
妇女在怀孕期间牙龈发炎的机率增加。有些研究甚至认为早产儿和新生儿体重过轻都与怀孕期间的牙龈疾病有关。虽然并非所有的研究都能提出具体证据支持以上的论点,但是怀孕过程中最好还是保持口腔健康,而且已有研究指出,妇女若能提早治疗牙龈疾病和改善口腔卫生,可以降低早产的风险。
倘若你已怀孕,请在你的产前护理项目中加入定期看牙医或牙周病医师。对于新手父母来说,维持口腔健康就是在身体力行地成为宝宝的最佳榜样。


(5)维持记忆力
根据神经病学、神经外科与精神病学期刊(Journal of Neurology, Neurosurgery & Psychiatry)的一项研究显示,相较于牙龈和牙齿健康的成年人,患有牙龈炎(牙龈肿胀、出血)的成年人在记忆力和其它认知功能的测试上表现较差。日常生活中每日都会使用的字词延宕回忆(delayed verbal recall)和减法这两种技能,牙龈炎患者在测验中表现较差。


(6)降低感染和发炎的风险
口腔不健康可能引起人体其它部位的感染。研究显示,口腔卫生不良可能引发老年人的肺炎。口腔内的细菌可能跑到肺部,引起肺部感染或导致肺部健康情况日益恶化。其它研究发现牙龈疾病和类风湿性关节炎之间的关连,类风湿性关节炎是一种引发关节发炎的自体免疫性疾病。专家认为,牙龈炎和类风湿性关节炎对结缔组织的损伤是相似的。
所以均衡饮食、定期检查牙齿和保持口腔卫生便能减少蛀牙,降低罹患牙龈疾病的风险。每日至少刷牙两次,使用牙线洁牙一次。此外使用抗菌漱口水或牙膏能减少口腔内引起牙龈炎的细菌。

If it is not removed, it can harden and become tartar. You cannot remove tartar with brushing and flossing. If plaque and tartar build up on your teeth, they can cause oral diseases.

Next, your gums will be checked. This will be done with a special tool to measure the spaces between your teeth and gums. With healthy gums, the spaces are shallow. When people have gum disease, the spaces may become deeper.

The check-up should also include your tongue, throat, face, head, and neck. This is to look for any signs of trouble, swelling, or cancer.

The cleaning

Brushing and flossing help clean the plaque from your teeth, but you can’t remove tartar at home. During the cleaning, your dental professional will use special tools to remove tartar. This is called scaling.

After your teeth are scaled, they may be polished. In most cases, a gritty paste is used for this. It helps to remove any surface stains on your teeth.

The final step is flossing. Your dental professional will use floss to make sure the areas between your teeth are clean.

What you should do between visits

Be sure to take care of your teeth and gums between regular dental visits. Plaque is always forming on your teeth, but you can get rid of it by brushing and flossing regularly. Here are some tips for good oral care at home.

  • Brush your teeth at least twice a day. Be sure to use a toothpaste that contains fluoride.
  • Floss at least once a day.
  • Use a mouth rinse to help get rid of plaque bacteria. This will also help to freshen breath.
